Lewandowski was forced off with a thigh injury in Monday's 2-1 win over Turkey, and is now a doubt for Poland's Euro 2020 opener against the Netherlands on Sunday.
The 32-year-old had already missed Poland's opening warm-up match against Russia due to a minor knock, but his latest injury is a major blow to his country's hopes of success at the tournament.
Lewandowski is Poland's all-time leading scorer and is considered one of the best strikers in the world. His absence will be a huge loss for Poland, who are already without injured forward Arkadiusz Milik.
Poland coach Paulo Sousa will now have to decide whether to start Krzysztof Piatek or Karol Swiderski in Lewandowski's place against the Netherlands.
